Notes:

Are you under the influence of mind-altering drugs or alcohol? Mind-altering drugs includes mind-altering prescription drugs as well as street drugs. Prescription medications for high blood pressure or diabetes are not mind-altering, while anti-depressants, stimulants etc. are. Coffee is OK in moderation.
