The name of the contact procedure
The time at the start of this procedure
When you have executed a command, or set of commands
Any comments as appropriate
For example:

6:18 pm
Poke and Receive

Notes:

Be receptive

"Be receptive to your loved one" means be very calm and open and willing to accept communication of any kind from your loved one. There is no mandatory time period for you to wait, but keep it somewhat balanced with the alternate poke command.

When you have done this, say something aloud, at least "Yes." Mark the worksheet when you have executed the command. It is not necessary for you to receive an actual communication in order for you to have executed the command, but you must have been willing for it to happen.
